I, the undersigned, hereby consent to receiving a tattoo from a licensed tattoo artist from SoulFire Tattoo Studio. I acknowledge, understand, and agree to the following:

1. Voluntary Participation

I am voluntarily choosing to receive a tattoo and am not under the influence of alcohol, drugs, or any impairing substance. I understand that receiving a tattoo

is a personal choice and that I may refuse service at any time.

2. Age & Identification

I confirm that I am 18years of age or older. I agree to present a valid

government-issued photo ID prior to receiving a tattoo.

3. Health Disclosure

I certify that I do not have any condition that may impair healing or increase risk, including but not limited to:

• Hemophilia or bleeding disorders

• Compromised immune system

• Diabetes

• Allergies to latex, pigments, or adhesives

• Skin conditions or infections at or near the tattoo site

I agree to notify the artist of any medical concerns prior to the procedure.

4. Risks of Tattooing

I understand that tattooing involves risks, including but not limited to:

• Pain or discomfort

• Infection

• Scarring

• Allergic reactions

• Ink spreading or fading over time

• Variations in appearance due to skin tone or healing

I acknowledge that solid red pigments, including brand-specific colors, may heal inconsistently depending on skin tone and long-term pigment retention.

5. Aftercare

I agree to follow the aftercare instructions provided to me. I understand that improper aftercare may result in infection, fading, or other complications.

6. Release of Liability

To the fullest extent permitted by Illinois law, I hereby release and hold

harmless:

• SoulFire Tattoo Studio (Ignited Soul LLC)

• Participating artists from any and all claims, damages, injuries, or expenses arising from or related to the tattoo and the procedure, including aftercare and healing.

This release applies to negligence except in cases of willful or wanton

misconduct, as defined under Illinois law.

7. Photography & Event Content

I consent to the tattoo and surrounding environment being photographed or filmed for event documentation and promotional use by SoulFire Tattoo

Studio.

8. Acknowledgment & Signature

I have read and fully understand this waiver. I have had the opportunity to ask questions. I agree to all terms voluntarily.
